Out-of-Network
Many health plans offer out-of-network benefits. Check with your insurance provider to see if your plan includes such benefits. We ask for payment at the time of your appointment. After each visit, we provide a completed claim form, with itemized fees and diagnoses. You simply submit the form to your insurance company for reimbursement.
